Athlete Information Form
Picture Day is near! Your athlete will have individual and team photos taken. Please complete this form before your athlete’s scheduled session to help make the ordering process more efficient. Once the photos are ready, we’ll send you a quick email or text with a link to view and order them online.
We love sharing moments from our sessions! iCreate Art Studio may use team and individual photos in our marketing, including on our website and social media platforms. If you’d prefer we don’t use your athlete’s individual photo, just let us know in the ‘Additional Information’ section. Team photos will still include all athletes.
Lastly, by sharing your phone and email, you agree to receive messages from us regarding your session. Your info stays private and will never be sold.
